P-014 SURGICAL TREATMENT OF COMPLEX INCISIONAL HERNIAS

Abstract Aim to study the early and long-term outcomes of abdominal wall reconstruction (AWR) complex incisional hernias. Material & Methods We retrospectively reviewed prospectively collected data from 121 patients with 1 7 years follow-up, who underwent midline AWR between 2015 2022. The complexity hernia was determined according criteria N. J. Slater et all. All had a gate width ≥10cm (W3). “Loss domain”≥20% 38% patients, recurrence after previously performed mesh-reinforced (R1–5) - 36%, purulent fistulas, trophic ulcers chronic seromas 9%. Three laparotomies or more in anamnesis 48.7% patients. Obesity 74.4%; type II diabetes mellitus 26.5%; COPD 15.7%. Mesh-reinforced fascial repairs were used 73 (60.33%) cases, bridged 48 (39.67%) cases. Results Wound complications postoperative period observed (40.5%) Long-term outcomes: (5.8%), ligature fistula 3 (2.5%), wound 2 (1.6%), pseudocyst building mesh (5.8%). AWRs primary coaptation resulted fewer recurrences (2.7% vs 10.4%) (31.5% 52.2%) overall (5.5% 16.6%) than repairs. At same time, pain (9.6% versus 4.2%) AWRs. Conclusions Surgical treatment hernias requires use combined techniques for using mesh, which reduces 5.8%.
